Strictly speaking, the bell (ghaNTa or tirumaNi) is rung when opening
and closing the doors of the sannidhi or box which contains the mUrti,
when beginning the pUja proper, whenever arghya is offered, for the
tirumanjanam, for dhUpa and dIpa, when offering food to the mUrti, 
including the parisecana and ApoSanas, and for nIrAjanam.  There are
some subtleties as to when the bell should be rung on both sides
and when only on one side (clang-clang vs. clang), but many if not
most, even among the orthodox, do not observe such detail.

The bell should have the right nAda.  As pointed out by Sri Rangapriya
Swami, the shastras stipulate that an ideal bell will have
the sound of the praNava when rung.  This nAda removes ajnAna 
(antar-dhvAnta) from the mind and is akin to the sound at the
gates of Vaikuntha.

Some Vaidikas also have ghaNTa-pratishThA done where the requisite
powers are invoked into the bell and it becomes officially
sanctified.  This is rare, however.
