Question on Gita recitation

Dear Bhagawathas,
   namaskArams.
   I would like to know the Gita dhyAna SlOkam recited by Sri Vaishnavas. I
guess
the dhyAna SlOkam starting as "pArthAya prathibhOdhithAm bhagawathA
nArayaNena swayam..."
is Sri Sankara's ?
   Also can we recite non consecutive chapters of Gita during the same
prayer session?
